Should you wish to refer a case then either phone Debbie, our receptionist, for a Referral Fax Back Form or download one here. Debbie will then make the appointment with the client directly and inform you when that appointment is. We will also send out an information pack to the clients about how to find us and what to expect when they come for their appointment. Please send any history, radiographs and laboratory reports before the client's appointment.
